Job Details:
Occupational Therapist required to provide an outcome-focused assessment service for adults with physical disabilities and age-related conditions.
Deliver a professional Occupational Therapy and Reablement service, including statutory social care responsibilities.
Carry out person-centred, strengths-based assessments to promote independence and recovery.
Work collaboratively with multidisciplinary teams and partner agencies to support individuals with complex needs.
Support safe and timely hospital discharges in partnership with health colleagues.
Maintain accurate and up-to-date case records in line with service standards.
Skills / Experience:
Qualified Occupational Therapist with HCPC registration.
Experience working with adults with physical disabilities and age-related conditions.
Experience within a social care or community setting.
Knowledge of reablement and outcome-focused practice.
Strong communication and multidisciplinary working skills.
