Job Details:
Occupational Therapist required to work within a multidisciplinary team including Social Workers, Occupational Therapists and Physiotherapists
Provide integrated health and social care services to the local community
Care coordinate a caseload of individuals with varying levels of complexity
Lead on coordinating care and support for service users and their carers/families, involving other professionals where appropriate
Support colleagues with complex cases where required and directed by the Team Manager
Maintain accurate and comprehensive case management records for monitoring and planning purposes
Provide an efficient and cost-effective service using appropriate resources from Social Services, Health, Council services and community organisations
Adhere to Health and Care Professions Council (HCPC) standards of proficiency
Maintain Continuous Professional Development (CPD) and attend post qualification training as required
Work flexibly where required to meet the needs of the service, potentially including a 7-day service model
