Job Title: Social Care Assistant

Job Details:
Social Care Assistant required (suitable for Occupational Therapy Assistant)
Assess the needs of adults and older people with physical disabilities, mental health needs and/or learning disabilities, as well as their informal carers
Undertake assessments in accordance with the Care Act and eligibility criteria
Work collaboratively with health and social care professionals to support an integrated approach to service delivery
Develop and implement strengths-based support plans to maximise independence and community living
Assess activities of daily living and arrange appropriate solutions including minor adaptations, assistive equipment and Assistive Technology
Screen for major adaptations and support moving and handling equipment needs
Support service users and carers to identify and commission appropriate resources, including direct payments and private provision
Provide ongoing assistance to individuals managing rehabilitation and complex support needs
Working Pattern: 2 days in the office, 3 days remote
