Job Title: Reablement Officer
Location: London Borough of Newham

Contract Length: Ongoing (3 months initially)
Job Details:
Reablement Officer required to start ASAP within the Adult Reablement Team
Initial 3-month contract
Support adults to regain independence and reduce or prevent ongoing care needs
Deliver structured reablement and/or rehabilitation programmes in individuals’ own homes
Work as part of an integrated intermediate care/reablement multidisciplinary team
Contribute to clear, accurate and person-centred care and support planning documentation
Maintain high-quality records in line with statutory and organisational requirements
Car driver preferred
Hybrid working – 2–3 days per week for visits
Essential Skills/Experience:
Experience in Adult Social Care Reablement and/or Rehabilitation
Proven experience working effectively within a Multidisciplinary Team (MDT)
Strong communication skills and ability to collaborate across professional boundaries
Experience contributing to person-centred care planning and documentation
Confident IT user, ideally with experience in health and/or social care systems
