Physiotherapist required within Therapies Medicine for an acute setting (DMOPS/Medicine AUH site)
Undertake timely physiotherapy assessments on DMOPS and Medicine wards
Determine discharge readiness and required physiotherapy interventions
Participate in parallel discharge planning
Develop and monitor individualised exercise and rehabilitation programmes
Liaise with hospital, community and multidisciplinary teams to coordinate care
