Job Title: Reablement Assistant

Job Details:
Reablement Assistant required
Support service users to meet physical, social and emotional needs in line with agreed care plans
Promote independence and enhance daily living skills
Must hold an up-to-date classroom Moving & Handling certificate (online only not accepted)
Assist service users with tasks outlined in individual support plans
Work with individuals to improve and maintain daily living skills using appropriate equipment where required
Liaise with other professionals to minimise risk and enable individuals to remain in their own homes
Work as part of an integrated team of allied health and social care professionals
Participate in assessments and reviews as required
Promote dignity, empowerment, independence and respect for cultural, spiritual and religious needs
Maintain accurate records and report significant changes to the Line Manager
Attend supervision sessions and contribute flexibly to changing service needs
Support service users in activities and recreational pursuits
Carry out duties in accordance with City Council policies and procedures
Working Hours: 35 hours per week, Monday to Friday, 09.00 – 16.00
