Job details: Occupational Therapist required to support a Double to Single Handed Care project, reviewing existing care packages and identifying opportunities to maximise independence and ensure safe, proportionate care provision.
Review double handed care packages with a focus on safe reduction to single handed care where appropriate
Undertake functional assessments within service users’ homes
Complete moving and handling assessments and recommendations
Recommend appropriate equipment and minor adaptations to support independence
Work collaboratively within a multidisciplinary team to promote strengths-based practice
Essential skills/experience:
Qualified Occupational Therapist with HCPC registration
Experience within Adult Social Care
Strong adaptations experience
Experience reviewing care packages and promoting independence
