Occupational Therapist required to start ASAP within the Community Rehabilitation Service.
This is a community-based role supporting older people in their own homes. Some remote working may be available for administrative tasks; however, the post holder must be able to drive and complete daily community visits.
Assessment and management of older people within their own homes
Delivery of rehabilitation and reablement interventions in the community
Completion of moving and handling assessments (essential)
Prescription of equipment to support independence and safety
Experience with minor adaptations required
Community-based caseload management
Car driver essential with access to own vehicle for work purposes
Working hours: Up to 37.5 hours per week, Monday to Friday
