Job details: D2A Occupational Therapist required. Hybrid working.

Deliver Discharge to Assess (D2A) Occupational Therapy interventions
Work collaboratively with health and social care colleagues to support safe and timely discharges
Undertake assessments and provide recommendations to promote independence
Operate within a hybrid working model, balancing remote and on-site work

Working hours: 37 hours per week. Monday to Friday, 09.00 – 17.30.
