Brief Job Overview

NELFT is seeking an experienced Band 6 Specialist Physiotherapist to join the Intensive Rehabilitation & Therapy Service based at Grays Court Community Hospital. This community rehabilitation role focuses on independent assessment, treatment, and discharge planning for patients with a range of rehabilitation needs.

This is a suitable opportunity for a confident clinician who can manage their own caseload and contribute to high-quality, patient-centred rehabilitation within a community NHS setting.
 

Key Responsibilities

•  
  • Undertake independent physiotherapy assessment and treatment within a community rehabilitation team.
     

  • Deliver evidence-based rehabilitation programmes using recognised outcome measures and strong clinical reasoning.
     

  • Lead on discharge planning to support safe and timely patient transitions.
     

  • Plan and prioritise your own caseload effectively.
     

  • Work collaboratively with the multidisciplinary team to optimise patient outcomes.
     

  • Maintain accurate and up-to-date clinical documentation in line with NHS standards.

    • • Degree or equivalent qualification in Physiotherapy.
         

      • Current HCPC registration.
         

      • Post-qualification experience within the NHS.
         

      • Experience in community and/or rehabilitation settings (preferred).
         

      • Ability to manage an independent caseload.
         

      • Valid UK driving licence and access to a vehicle (essential).

       

      Contract & Working Pattern

    •  
      • Locum contract – ongoing
         

      • 37.5 hours per week
         

      • Monday to Friday
         

      • 08:30 – 16:30
         

      • Competitive rate – £negotiable
