NELFT is seeking an experienced Band 6 Specialist Physiotherapist to join the Intensive Rehabilitation & Therapy Service based at Grays Court Community Hospital. This community rehabilitation role focuses on independent assessment, treatment, and discharge planning for patients with a range of rehabilitation needs.
This is a suitable opportunity for a confident clinician who can manage their own caseload and contribute to high-quality, patient-centred rehabilitation within a community NHS setting.
Key Responsibilities

- Undertake independent physiotherapy assessment and treatment within a community rehabilitation team.
- Deliver evidence-based rehabilitation programmes using recognised outcome measures and strong clinical reasoning.
- Lead on discharge planning to support safe and timely patient transitions.
- Plan and prioritise your own caseload effectively.
- Work collaboratively with the multidisciplinary team to optimise patient outcomes.
- Maintain accurate and up-to-date clinical documentation in line with NHS standards.

- Degree or equivalent qualification in Physiotherapy.
- Current HCPC registration.
- Post-qualification experience within the NHS.
- Experience in community and/or rehabilitation settings (preferred).
- Ability to manage an independent caseload.
- Valid UK driving licence and access to a vehicle (essential).
Contract & Working Pattern
- Degree or equivalent qualification in Physiotherapy.

- 37.5 hours per week
- Monday to Friday
- 08:30 – 16:30
- Competitive rate – £negotiable
