Job details: Enthusiastic and motivated Physiotherapist required to start asap for 2 months. Urgent Community Response (UCR) and Discharge Service (AADS) – Frailty Assessment Clinic.

 

This specialist role is part of the Urgent Community Response Service, working across community and hospital settings to facilitate early discharge and prevent unnecessary hospital admissions. The service uses the Discharge to Assess model and works closely with social care partners, including Reablement services and hospital social workers. The role includes rotation within the UCR team covering Rapid Response, Falls Pick-Up Service, Intermediate Care, hospital discharge support, and Admission Avoidance in the Emergency Department.

 

  •  
    • Work with partners across health and social care to ensure an integrated, joined-up service
       

    • Identify patients at risk of repeated admission and implement preventative measures
       

    • Provide rapid response, specialist MDT triage within the Emergency Department and community
       

    • Deliver physiotherapy assessments for patients with complex physical, cognitive and psychological needs
       

    • Develop client-centred treatment plans addressing physical, social and environmental needs
       

    • Work collaboratively with community health services and Emergency Department staff
       

    • Provide urgent community follow-up following ED discharge to reduce admission risk
       

    • Support Admission Avoidance services in the community as required
       

    • Contribute to ongoing service development within admission avoidance services
       

    • Deputise for the Band 7 Therapist when required, managing allocated operational responsibilities

     

    Skills/experience: Previous experience delivering both acute and community-based physiotherapy assessments and interventions.

     

    Working hours: 0.4 WTE
