Intensive Rehab & Therapy Service (Winter Pressures Support)

Role Overview

We are seeking an experienced Band 6 Physiotherapist to join the Intensive Rehab & Therapy Service at Grays Court Community Hospital.  
You will work as an autonomous practitioner, delivering independent assessment, treatment, and discharge planning for patients within a community rehabilitation setting.

This role is ideal for a confident clinician who can manage and prioritise their own caseload while using recognised outcome measures and clinical reasoning to deliver effective rehabilitation.
 

Key Responsibilities

  • Undertake comprehensive assessments and formulate individualised treatment plans.
  • Provide high-quality rehabilitation to support patient recovery and independence.
  • Lead on discharge planning and liaise with wider MDT services.
  • Use recognised outcome measures to evaluate progress and guide interventions.
  • Prioritise and manage a varied caseload independently.
  • Contribute to safe, timely, patient-centred care during winter pressure periods.
  • Maintain accurate documentation and comply with NHS standards and policies.

About You

  • HCPC registered Physiotherapist.
  • Post-qualification experience — preferably within community and rehabilitation services.
  • Strong clinical reasoning and assessment skills.
  • Confident working autonomously and as part of an MDT.
  • Excellent communication and discharge planning ability.
  • Car driver (essential) due to community travel.

Working Hours

  • Monday to Friday
  • 08:30 – 16:30
  • 37.5 hours per week
  • Shift-based working
