About the Role

 

An opportunity has arisen for an experienced Band 6 Occupational Therapist to join the Community Therapy Service. This role involves delivering high-quality, person-centred occupational therapy within patients’ homes, supporting functional independence and helping to reduce avoidable hospital admissions.

 

Main Responsibilities

 

  • Complete comprehensive occupational therapy assessments within patients’ homes, including functional and cognitive assessments.
  • Develop and implement intervention plans to improve function and promote independence.
  • Assess, recommend and provide appropriate equipment to reduce risk and support safe living.
  • Work collaboratively within a multidisciplinary team including Physiotherapists, Therapy Assistants and Social Workers.
  • Contribute to care planning and ongoing review of patients within the community.
  • Maintain accurate and timely clinical documentation in line with Trust and HCPC standards.

 

Skills & Experience Required

 

  • Degree in Occupational Therapy and HCPC registration.
  • Experience working as a Band 6 Occupational Therapist.
  • Community-based occupational therapy experience.
  • Strong assessment, clinical reasoning and MDT working skills.
  • Experience with equipment provision and risk reduction in the home environment.

 

Working Hours

 

  • 37.5 hours per week.
  • Monday to Friday, 08:30 – 16:30.
