About the Role
Medway NHS Foundation Trust is seeking a Band 6 Physiotherapist to join the Older Persons / Elderly Frailty service. This role focuses on delivering high-quality physiotherapy assessment and intervention to older adults, supporting mobility, rehabilitation and safe discharge planning within an acute hospital setting.
Main Responsibilities
Provide physiotherapy assessment and treatment for older adults with frailty and complex needs.
Develop and implement rehabilitation and mobility programmes tailored to individual patient needs.
Support falls prevention, functional recovery and safe discharge planning.
Work collaboratively within the multidisciplinary team to optimise patient outcomes.
Maintain accurate and timely clinical documentation in line with Trust and HCPC standards.
Skills & Experience Required
HCPC-registered Physiotherapist.
Experience working with older people and/or frailty services.
Strong assessment, clinical reasoning and rehabilitation skills.
Ability to work effectively within a multidisciplinary team.
Working Hours
Shift pattern: 08:00 – 16:00.
