Location: City Health Care Partnership CIC – Longhill Health Centre

Area: East Riding of Yorkshire NHS

Rate: £Negotiable

Contract: Locum, Ongoing

 

About the Role

 

City Health Care Partnership CIC is seeking an experienced Band 6 Physiotherapist to join their community stroke service. This locum role is initially for 3 months and involves delivering specialist stroke rehabilitation across Hull and the East Riding, supporting individuals in their homes and community settings.

 

Main Responsibilities

 

  • Provide specialist physiotherapy assessments and interventions for adults recovering from stroke.
  • Work autonomously across Hull and East Riding, managing a diverse community caseload.
  • Develop tailored rehabilitation programmes aimed at improving mobility, function and independence.
  • Collaborate with multidisciplinary colleagues to ensure coordinated and effective care.
  • Maintain accurate clinical notes in line with HCPC and service requirements.
  • Contribute to the ongoing delivery of a high-quality, community-based stroke rehabilitation service.

 

Requirements

 

  • Degree in Physiotherapy (post-graduate qualification and 3+ years of experience desirable but not essential).
  • Experience in community-based stroke rehabilitation.
  • Strong assessment and clinical reasoning skills.
  • Car driver with own vehicle, willing to travel across Hull and East Riding.
  • SystmOne experience desirable but not essential.
  • Ability to work independently and manage a varied community caseload.

 

Working Hours

 

  • Monday to Friday, 08:00 – 16:00.
