Brief Job Overview

 
We are seeking a motivated and experienced Community Occupational Therapist (Band 6/7) to join the Walton Centre NHS Foundation Trust in Liverpool. This is an exciting locum opportunity for a therapist with a strong background in community rehabilitation, neurorehabilitation, or complex discharge planning.  

The successful candidate will play a vital role in supporting patients within the community to regain independence, promote recovery, and prevent hospital readmissions. You will work collaboratively with multidisciplinary teams across health and social care to deliver holistic, person-centred care to individuals with complex physical or neurological conditions.  
 

Key Responsibilities

 

  • Provide comprehensive community-based assessments for patients with neurological and complex physical health needs.
  • Develop and deliver goal-orientated intervention plans to support recovery, independence, and functional improvement.
  • Work collaboratively with GPs, nurses, physiotherapists, social workers, and rehabilitation teams to coordinate care and support timely discharge from hospital.
  • Recommend and facilitate access to appropriate equipment, assistive technology, and home adaptations to promote safe and independent living.
  • Contribute to rehabilitation and reablement programmes within patients’ homes or community settings.
  • Provide education, training, and advice to patients, carers, and family members to support ongoing self-management and independence.
  • Maintain accurate clinical records and ensure all documentation complies with NHS and HCPC standards.
  • Participate in multidisciplinary team meetings, service planning, and clinical audits to support continuous improvement.

 

About You

 
You are a compassionate, adaptable, and proactive Occupational Therapist with solid experience in community rehabilitation or neurorehabilitation. You are skilled at working autonomously while maintaining strong collaboration with other professionals to deliver safe, effective, and high-quality care.  

Your approach is person-centred and strength-based, with a focus on enabling individuals to live fulfilling, independent lives.  
 

Requirements

 

  • BSc (Hons) or Diploma in Occupational Therapy.
  • HCPC registration as an Occupational Therapist.
  • Significant post-qualification experience in community, neuro, or physical rehabilitation settings.
  • Experience with complex case management, equipment prescription, and discharge planning.
  • Strong knowledge of NHS pathways, rehabilitation principles, and relevant legislation.
  • Excellent communication and interpersonal skills for working across multi-agency settings.
  • Ability to prioritise workload, manage time effectively, and work both independently and as part of a team.
  • Access to a vehicle and valid driving licence (preferred for community travel).

 

Contract & Working Pattern

 

  • Start Date: February/March 2026
  • Working Hours: Likely part-time (to be confirmed)
  • Days: Monday – Friday (some flexibility may be required)
  • Contract: Ongoing locum position
