Brief Job Overview
We are seeking an experienced and dedicated Occupational Therapist to join the Adult Social Care HUB within Bracknell Forest Borough Council. This locum position offers a rewarding opportunity to work within an established multi-professional statutory team, supporting adults with a diverse range of needs to live independently and safely within their communities.
As part of the Adult Social Care HUB, you will be the first point of contact for residents with physical health issues, learning disabilities, or age-related conditions. Your assessments and interventions will help improve functional ability, promote independence, and enhance overall wellbeing.
Key Responsibilities
- Undertake comprehensive assessments of adults with physical, cognitive, or learning disabilities to determine their care and support needs.
- Develop and implement person-centred care plans designed to maximise independence and quality of life.
- Advise on, arrange, and review equipment, assistive technology, and home adaptations to meet identified needs.
- Support older adults, people with learning disabilities, physical disabilities, and dementia to achieve personal goals and maintain daily living skills.
- Collaborate effectively with colleagues across health, housing, and the voluntary care sector to deliver coordinated, holistic support.
- Ensure all practice aligns with the Care Act 2014 and other relevant legislation.
- Promote inclusion, independence, and wellbeing through strengths-based and solution-focused approaches.
- Maintain accurate records and reports in accordance with council and professional standards.
About You
You are a proactive, compassionate, and skilled Occupational Therapist who thrives on empowering individuals to live independently. You will bring sound clinical reasoning, strong assessment skills, and the ability to develop creative, person-centred interventions.
You will enjoy working within a multi-disciplinary setting, using your expertise to support individuals with a wide range of needs — from physical disabilities to complex neurological or cognitive conditions.
Requirements
- BSc (Hons) or Diploma in Occupational Therapy.
- HCPC registration as an Occupational Therapist.
- Significant post-qualification experience in Adult Social Care or community-based settings.
- Experience working with individuals with learning disabilities, dementia, or age-related health needs.
- Strong working knowledge of the Care Act 2014 and relevant social care legislation.
- Excellent communication, problem-solving, and documentation skills.
- Ability to work autonomously while contributing effectively to a multi-disciplinary team.
- Commitment to promoting independence, dignity, and person-centred care.
Contract & Working Pattern
- Working Hours: Full-time, 37.5 hours per week
- Days: Monday – Friday
- Contract: Ongoing locum position
