Social Worker – Preparing for Adulthood Team (Bromley Council)
Job Overview
We are recruiting a committed **Qualified Social Worker (QSW)** to join the **Preparing for Adulthood Team at Bromley Council**. This role focuses on supporting young people with additional needs as they move from children’s to adult services, ensuring their independence, wellbeing, and life opportunities are promoted throughout the transition process.
As a Social Worker in this team, you will manage a caseload of young people aged 14–25, work closely with families and partner agencies, and help create individualised plans that support aspirations around education, employment, health, and independent living.
Key Responsibilities
Undertake assessments for young people preparing for adulthood, identifying strengths, risks, and support needs.
Develop and review person-centred transition plans that focus on independence and positive life outcomes.
Support young people and their families through the transition from children’s to adult social care services.
Work collaboratively with education providers, health services, housing, and voluntary organisations to ensure coordinated multi-agency support.
Manage a varied caseload, ensuring statutory duties are met within timescales.
Prepare and present clear, evidence-based reports for reviews, panels, and legal processes where required.
Promote young people’s rights, independence, and participation in decision-making.
Maintain accurate and timely case recording in line with legislation and Bromley Council policies.
About You
You are a dedicated and resilient social worker with a passion for supporting young people with additional needs. You have strong assessment and planning skills, a child- and young person-centred approach, and experience of working with transition services or safeguarding.
Requirements
Qualified Social Worker (QSW) with registration to **Social Work England**.
Experience working with young people, ideally in transition or preparing for adulthood services.
Strong knowledge of safeguarding, the Children Act, and the Care Act.
Excellent assessment, planning, and communication skills.
Ability to work effectively within a multi-agency environment.
