  • Undertake safeguarding enquiries under Section 42 of the Care Act, ensuring risks are identified and managed effectively.
  • Work collaboratively with individuals, carers, families, and partner agencies to coordinate appropriate services and support.
  • Ensure service users’ voices are central to the assessment and planning process, promoting choice, dignity, and control.
  • Carry out reviews of existing care plans, ensuring they remain appropriate and proportionate to individuals’ changing needs.
  • Apply the principles of the Mental Capacity Act 2005 and the Care Act 2014 in everyday practice.
  • Maintain accurate and timely records in line with professional and council standards.
  • Participate in supervision, reflective practice, and professional development activities to maintain high-quality standards of practice.

About You

You are a committed and experienced social worker who is passionate about supporting adults to achieve independence, dignity, and wellbeing. You bring excellent assessment and communication skills and thrive in a fast-paced, multi-agency environment. You are confident in making decisions, managing risk, and ensuring that adults receive the right care at the right time.
