Brief Job Overview

 
We are seeking an experienced and dedicated Adult Social Worker to join the Adult Social Care Service at the London Borough of Waltham Forest on a 3-month locum contract. This role is ideal for a qualified professional who is passionate about supporting adults to live independently, promoting wellbeing, and delivering high-quality person-centred social work practice.  

You will work within a supportive and collaborative environment, undertaking assessments, developing care plans, and coordinating services that empower individuals to maintain independence and safety in their communities.

Key Responsibilities

• Undertake holistic assessments under the Care Act 2014 to identify needs, strengths, and desired outcomes for adults requiring support.
• Develop person-centred care and support plans that promote independence, wellbeing, and choice.
• Manage a diverse caseload including older adults, individuals with physical disabilities, learning disabilities, or mental health needs.
• Work in partnership with health professionals, housing providers, and voluntary sector organisations to ensure effective multi-agency care coordination.
• Undertake Safeguarding Adults enquiries in line with statutory duties and local safeguarding procedures.
• Support hospital discharges and community-based interventions to ensure timely and safe transitions of care.
• Apply knowledge of the Mental Capacity Act 2005 and other relevant legislation to promote individuals’ rights and choices.
• Maintain accurate and up-to-date case records, ensuring compliance with local authority policies and national standards.
• Participate in supervision, reflective practice, and service development initiatives to enhance professional learning and performance.

Requirements

• Qualified Social Worker registered with Social Work England.
• At least two years’ post-qualified experience in adult social care.
• Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults procedures.
• Experience working with adults with physical disabilities, learning disabilities, or mental health conditions.
• Excellent assessment, care planning, and risk management skills.
• Strong communication and interpersonal skills, with the ability to build effective professional relationships.
• Commitment to promoting independence, choice, and dignity for adults.
• Ability to work both independently and collaboratively within a multi-disciplinary team.
