Social Worker – Complex Care (ASW)

Brief Job Overview

 
We are recruiting an experienced and dedicated Social Worker to join the Complex Care Team at the London Borough of Bexley on a 3-month locum contract. This is a key role for a skilled practitioner with a minimum of three years’ post-qualified experience and strong expertise in Continuing Healthcare (CHC) assessments.  

You will be part of a collaborative, multi-disciplinary team delivering person-centred, outcome-focused interventions that promote independence, choice, and wellbeing for adults with complex health and social care needs.

Key Responsibilities

• Undertake comprehensive Continuing Healthcare (CHC) assessments and reviews, ensuring accurate application of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care.
• Manage a caseload of adults with complex needs, developing person-led care and support plans that promote independence and reduce risk.
• Work collaboratively with health professionals, providers, and partner agencies to ensure holistic and integrated care planning.
• Provide expert advice and guidance on CHC eligibility and funding processes, ensuring decisions are made in line with national guidance and best practice.
• Undertake Care Act assessments, safeguarding enquiries, and Mental Capacity Act assessments as required.
• Contribute to multidisciplinary team meetings and case discussions, ensuring effective information sharing and joint decision-making.
• Maintain accurate, timely case records and reports in accordance with statutory duties and local authority policy.
• Balance caseload priorities while ensuring high-quality, person-centred outcomes.

About You

 
You are a confident and knowledgeable social worker with a strong background in adult social care, particularly in complex care and CHC assessment work. You are analytical, thorough, and able to apply professional judgement to complex decision-making. You thrive in a collaborative environment and are committed to improving outcomes for adults through high-quality, evidence-based practice.

Requirements

• Qualified Social Worker registered with Social Work England.
• At least three years’ post-qualified experience within adult social care, ideally in a complex care or hospital discharge setting.
• Strong knowledge and practical experience in CHC assessments and the National Framework for NHS Continuing Healthcare.
• Experience in Care Act assessments, safeguarding, and mental capacity work.
• Excellent communication, assessment, and analytical skills.
• Ability to work both independently and collaboratively within a multi-disciplinary team.
• Commitment to person-centred practice and promoting independence and wellbeing.
• Willingness to attend the office at least twice per week as part of hybrid working arrangements.
