Job Overview
We are looking for a skilled and adaptable Occupational Therapist to join North West London Social Services on a locum basis. This role offers a rewarding opportunity to work across two dynamic settings — supporting residents in both care homes and their own homes.
You will play a key role in promoting independence, safety, and wellbeing through thorough functional assessments, effective reablement interventions, and the provision of appropriate equipment and adaptations. This position is ideal for an experienced Occupational Therapist seeking a varied role that makes a meaningful difference in people’s daily lives.
Key Responsibilities

- Conduct functional assessments and develop personalised care and reablement plans to support independence and long-term wellbeing.
- Work three days per week within care homes as part of a Reablement Pilot OT service — assessing new and returning residents to optimise function and plan sustainable care support.
- Work two days per week based at a local Health Centre, undertaking community-based assessments in residents’ own homes.
- Recommend and arrange the provision of equipment and specialist aids to enhance safety, accessibility, and independence.
- Assess and advise on major adaptations, ensuring compliance with relevant policies and funding pathways.
- Carry out moving and handling assessments and provide training or guidance where required.
- Work collaboratively with multidisciplinary teams, including physiotherapists, social workers, and reablement staff, to deliver integrated care.
About You
You are a proactive and person-centred Occupational Therapist who enjoys working across varied environments. You combine strong clinical reasoning with a practical approach to problem-solving, helping people regain confidence and independence in daily living. A background in reablement or community-based OT work would be an advantage.
Requirements

- Degree or Diploma in Occupational Therapy with current HCPC registration.
- Previous experience in care home or community settings, ideally with reablement experience.
- Knowledge of equipment provision, major adaptations, and moving and handling assessments.
- Ability to manage a varied caseload independently and as part of a multidisciplinary team.
- Excellent communication, time management, and documentation skills.

- Location: North West London Social Services (across two sites)
- Pay rate: Negotiable (dependent on experience)
- Contract type: Ongoing
- Working pattern: 3 days in care homes, 2 days at Health Centre
