As an Occupational Therapist with HCRG Care Group, you will provide expert assessment and intervention within community settings, focusing on acute care, rehabilitation and reablement. Working at Band 6 level, you will deliver person-centred care to support service users in achieving maximum independence and quality of life. You will collaborate with multidisciplinary teams to ensure safe, effective and timely interventions.
Complete comprehensive occupational therapy assessments to identify individual needs and goals.
Plan and implement tailored rehabilitation and reablement programmes to promote independence.
Deliver acute interventions to support recovery and prevent hospital admissions.
Recommend and arrange appropriate equipment or adaptations to enhance safety and daily living.
Work closely with multidisciplinary colleagues, service users and carers to ensure coordinated care.
Maintain accurate clinical records in accordance with professional and organisational standards.
