Job Details:  Urgent and Emergency Care. Occupational Therapist or Physiotherapist required as Therapist Lead for Care Transfer Hub. Likely to be able to offer work 4-8 weeks. JD available on request:

  • As a senior clinician within the Care Transfer Hub (CTH) multi-disciplinary team you will work with a “home first” philosophy. You will be an advocate for the patient in determining the right pathway for them as individuals following a discharge to assess model.
  • You will work in collaboration with other health and social care professionals in an integrated way providing autonomous clinical, professional decision-making to support the planning of complex discharges.
  • The post holder will develop the ethos of multi-disciplinary, integrated care and seamless services within acute and or community hospitals ensuring that discharge planning is delivered to the highest possible standard.
  • The post holder will be working flexibly across both acute hospital sites and may be required to work in community settings as part of their role and participate in 7 day working when appropriate service delivery allows and commences. 

The postholder will:

  • Be an autonomous practitioner and manage a complex caseload as appropriate to the role in alignment with the agreement between the Operational Service Manager.
  • Management of patients will be based on, evidence-based approaches in line with NICE guidance.
  • Exercise autonomous professional responsibility for management and discharge of patients whilst working collaboratively in highly complex and contentious discharge planning situations.
  • Provide highly specialist advice regarding patient management and the discharge options available to the MDT patients and carers.
  • Support positive risk management, responding to and resolving problems to reduce risk, improve safety and enable least restrictive discharge planning
  • Demonstrate advanced skills in communicating complex or contentious information.
  • Demonstrate and apply a high level of understanding of the effects of disability, loss of function, loss of independence and frailty.
  • Act as a first line for staff clinical queries about referrals and risk issues.                 
  • Ensure the service operates in line with the Organisations policies and protocols procedures.
  • To develop and maintain professional working relationships with the Care Homes commissioned to provide care and support for patients discharged from the acute and community beds.

Working hours: Full time. 37.5 hours per week. Rota is Monday – Sunday (weekend work is part of the role)
