    Job Overview

    We are seeking experienced Advanced Practitioners and Occupational Therapists to join Brent Council within the North West London Social Services area. These locum positions form part of a fast-paced front-door service, focusing on effective and timely hospital discharge and community reablement. You will play a key role in ensuring that residents receive the right support at the right time, promoting safe transitions from hospital to home and preventing unnecessary readmissions. This is an excellent opportunity to contribute to a dynamic, multidisciplinary team committed to delivering person-centred, outcome-driven care.

    Key Responsibilities

       

    • Provide expert Occupational Therapy assessment and intervention to support timely and safe hospital discharges.
       

    • Work as part of a Discharge to Assess (D2A) or equivalent integrated team, ensuring residents’ needs are identified and supported promptly.
       

    • Develop, implement, and review care and support plans focused on independence, functional recovery, and wellbeing.
       

    • For the Advanced Practitioner role, hold a small caseload while providing leadership, guidance, and management support to Occupational Therapists and Occupational Therapy Assistants.
       

    • Conduct daily community visits to assess and review clients’ needs in their home environments, ensuring interventions are appropriate and effective.
       

    • Work collaboratively with NHS colleagues, hospital teams, social workers, and community partners to achieve seamless care transitions.
       

    • Maintain accurate, high-quality records and documentation in line with professional and council standards.

    About You

    You are an adaptable and proactive Occupational Therapist or Advanced Practitioner with a strong background in hospital discharge and reablement work. You thrive in fast-paced environments and are skilled in managing both clinical and leadership responsibilities. With excellent communication and problem-solving skills, you’ll be confident supporting individuals with a range of physical and functional needs while fostering collaboration within the multidisciplinary team.

    Requirements

       

    • Degree or diploma in Occupational Therapy and registration with the Health and Care Professions Council (HCPC).
       

    • Previous experience within hospital discharge, reablement, or Discharge to Assess (D2A) services.
       

    • Strong understanding of occupational therapy within adult social care and hospital-to-home pathways.
       

    • For Advanced Practitioner roles: experience in staff supervision, leadership, and team coordination.
       

    • Excellent assessment, clinical reasoning, and documentation skills.
       

    • Ability to work on-site 4–5 days per week and conduct at least two visits daily.
       

    • Previous NHS or local authority experience desirable.
