Advanced Practitioner & Occupational Therapist (2 Roles)
Job Overview
Brent Council is recruiting for two experienced locum professionals — an Advanced Practitioner Occupational Therapist and a Fieldwork Occupational Therapist — to join their fast-paced Hospital Discharge and Front Door Team. These roles are ideal for motivated OTs with strong clinical reasoning skills and a passion for supporting timely, safe hospital discharges and promoting independence in the community.
Key Responsibilities
– Complete assessments to facilitate safe and effective hospital discharges in line with the Discharge to Assess (D2A) model.
– Support service users to return home or transition to appropriate care settings, ensuring all equipment and support packages are in place.
– Work closely with health and social care colleagues to promote integrated, person-centred care.
– Manage a small caseload of complex discharge cases while contributing to overall team management and service delivery (Advanced Practitioner role).
– Provide supervision, mentoring, and professional guidance to Occupational Therapists and Occupational Therapy Assistants (Advanced Practitioner role).
– Conduct home visits and community assessments to ensure safe environments for discharge and ongoing independence.
– Maintain accurate documentation and adhere to professional and legislative standards.
About You
You’ll be an experienced and proactive Occupational Therapist with a strong understanding of hospital discharge and community reablement processes. You’ll thrive in a fast-paced environment, demonstrating excellent communication, prioritisation, and teamworking skills.
Requirements
– Degree or diploma in Occupational Therapy (or equivalent)
– Current registration with the Health and Care Professions Council (HCPC)
– Proven experience working in hospital discharge or D2A teams (essential)
– Previous NHS or local authority experience (desirable)
– Strong understanding of moving and handling, equipment provision, and risk assessment
– Ability to manage a varied caseload efficiently
– Advanced Practitioner role: supervisory and leadership experience within an OT team
