Job Overview

Shropshire Community Health NHS Trust is seeking an experienced Band 6 Physiotherapist to join its Urgent Community Response (UCR) and Virtual Locality Teams. This locum position plays a vital role in supporting admission avoidance services through rapid assessment and intervention of patients in community settings.  

 

You’ll work primarily across Telford, Shrewsbury, and Bridgnorth, with flexibility to support other localities across Shropshire as required. This is an excellent opportunity for a dynamic and adaptable physiotherapist who thrives in fast-paced, community-based environments.

 

Key Responsibilities

– Deliver rapid response physiotherapy assessments and interventions to prevent unnecessary hospital admissions.  

– Provide holistic, patient-centred care in homes and community settings, ensuring continuity across the care pathway.  

– Undertake physical assessments and basic clinical observations to inform treatment and care planning.  

– Work collaboratively within the multidisciplinary team (MDT) to ensure safe, effective, and coordinated patient care.  

– Contribute to service development and support the delivery of high-quality physiotherapy within the UCR model.  

– Maintain accurate clinical documentation in line with HCPC and NHS standards.  

 

About You

You’ll be a motivated and compassionate Physiotherapist with experience in community or acute care settings. You’ll demonstrate sound clinical reasoning, strong teamwork, and the ability to deliver rapid, responsive interventions that promote patient independence and recovery.  

 

Requirements

– Degree or equivalent qualification in Physiotherapy  

HCPC registration (essential)  

– Experience working within community or urgent care teams (desirable)  

– Excellent assessment, communication, and organisational skills  

– Ability to work flexibly across multiple localities as required  

Full UK driving licence and access to own vehicle (essential)

Working hours: 08:00 – 20:00, seven days a
