Deliver statutory social care responsibilities on behalf of North East Lincolnshire Council, completing Care Act 2014 assessments, reviews, and support planning for adults with eligible needs.
Manage a caseload of service users, including older people, individuals with learning disabilities, physical disabilities, or mental health conditions, ensuring timely and person-centred interventions.
Develop and implement care and support plans that promote independence, safety, and wellbeing, working collaboratively with service users, carers, and professionals.
Undertake safeguarding enquiries under Section 42 of the Care Act, assessing risk and coordinating multi-agency responses to protect adults at risk of harm or neglect.
Complete Mental Capacity Act assessments and contribute to best interest decisions, advocating for the rights and preferences of those who lack capacity.
Maintain accurate case records, reports, and assessments, while engaging in supervision and ongoing professional development to support high-quality practice across North East Lincolnshire’s Adult Services.
