Undertake statutory duties on behalf of Middlesbrough Borough Council, delivering Care Act 2014 assessments and reviews to support adults with care and support needs.
Manage a varied caseload, including older adults, individuals with learning disabilities, physical disabilities, or mental health issues, ensuring timely and person-centred interventions.
Develop and implement personalised care and support plans that promote independence, wellbeing, and safeguarding in line with local policies and national legislation.
Conduct safeguarding enquiries under Section 42 of the Care Act, working in partnership with health, housing, and other agencies to protect adults at risk.
Complete Mental Capacity Act assessments, contribute to best interest decisions, and support Continuing Health Care (CHC) processes when appropriate.
Maintain accurate, up-to-date case records and participate in regular supervision and training to uphold high standards across Middlesbrough Borough Council’s Adult Social Care services.
