Job Details:
Social Care Practitioner required to support a backlog of non-complex Occupational Therapy waiting list assessments. Candidates do not need to be a qualified Occupational Therapist, but must hold a Level 3 Diploma in Adult Care (or equivalent) and have experience working in a local authority environment.
The post holder will take a strength-based approach, working closely with clients, equipment providers, Housing and Health departments to enable independence at home through small aids, minor and major adaptations.
The role will be supported by a wider network including Social Care Practitioners, Occupational Therapists, Team Coordinators, and dedicated OT service managers and senior practitioners.
Key Responsibilities:

Undertake strength-based assessments of adults in the community.

Recommend and arrange equipment solutions, minor and major housing adaptations.

Support safe, proportionate, and personalised care delivery.

Maintain accurate and timely records of assessments and interventions.

Contribute to multi-agency and multidisciplinary approaches across the council and statutory bodies.

Travel to client homes as required.

Engage in supervision and professional development with experienced Occupational Therapists.
Skills & Experience Required:

Knowledge/experience of person-centred, holistic, and strength-based approaches.

Experience assessing activities of daily living.
-
Understanding of physical, learning disabilities, and/or mental health conditions and their impact on daily living.

Experience with equipment and housing adaptation solutions.

Ability to manage and prioritise casework.

Excellent communication and relationship-building skills with partners, professionals, and agencies.

Good IT and time management skills (experience with Mosaic system desirable).

Knowledge of statutory legislation underpinning OT assessment and interventions in adult social care.

Commitment to safeguarding principles and embedding them in practice.

Reflective practitioner with commitment to supervision and professional development.
