Act on behalf of Bristol City Council to carry out Care Act 2014 assessments, promoting independence, wellbeing, and personalised care for adults with diverse and complex needs.
Manage a varied caseload including older adults, individuals with physical or learning disabilities, and those with mental health conditions, ensuring timely and proportionate interventions.
Develop and review person-centred care and support plans, working collaboratively with service users, carers, families, and multi-agency professionals to deliver effective outcomes.
Undertake safeguarding enquiries under Section 42 of the Care Act, assessing risk and coordinating multi-agency responses to protect adults from harm, abuse, or neglect.
Maintain accurate and timely records and documentation, including reports for Continuing Health Care (CHC), Mental Capacity Act (MCA) assessments, and best interest decisions.
Participate in supervision, reflective practice, and ongoing professional development, contributing to a culture of continuous improvement within Bristol City Council’s Adult Social Care services.
