Undertake strengths-based assessments under the Care Act 2014 on behalf of Stockton-on-Tees Borough Council, focusing on promoting independence, wellbeing, and personal choice.
Manage a diverse caseload of adults with complex needs, including older people, individuals with physical or learning disabilities, and those experiencing mental health challenges.
Develop and implement person-centred care and support plans, working collaboratively with service users, carers, families, and multi-agency professionals.
Carry out safeguarding enquiries in line with Section 42 of the Care Act, ensuring timely and proportionate responses to concerns about abuse, neglect, or self-neglect.
Maintain accurate, high-quality case records and reports for panels, reviews, court proceedings, and Continuing Health Care assessments, in line with legal and council policies.
Actively participate in supervision, team meetings, and ongoing professional development, supporting continuous improvement and high standards within Adult Social Care at Stockton-on-Tees Borough Council.
