Join Bradford Metropolitan District Council as a senior practitioner to lead and oversee complex statutory assessments for adults, ensuring compliance with legislation such as the Care Act 2014.
Take responsibility for developing, implementing, and reviewing person-centred care and support plans within Bradford Metropolitan District Council’s framework, promoting independence, safety, and wellbeing.
Manage and prioritise a complex caseload, while providing leadership, mentorship, and informal supervision to junior social workers and trainees in the adult services team.
Lead safeguarding adults at risk processes for Bradford Metropolitan District Council, including coordinating safeguarding enquiries, attending strategy meetings, and contributing to safeguarding plans.
Collaborate strategically with multi-agency partners (health, housing, mental health, voluntary sector) to ensure coordinated and effective support for vulnerable adults.
Maintain accurate, timely case records and reports in accordance with Social Work England standards and Bradford Metropolitan District Council’s policies, contributing to continuous service improvement.
