Join a dedicated Adult Social Care team, delivering person-centred support to adults with care and support needs, promoting independence, dignity, and wellbeing.
Undertake statutory assessments and interventions under the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation, ensuring timely and proportionate responses.
Manage a varied caseload involving safeguarding enquiries, mental capacity assessments, hospital discharges, and care planning for individuals with complex needs.
Work in close partnership with service users, families, carers, and multi-agency professionals to provide holistic and coordinated support.
Apply strengths-based, outcome-focused practice, promoting choice and control for adults while supporting least restrictive options and community-based solutions.
