As an Occupational Therapist with East Sussex County Council, you will provide specialist assessment and intervention to support individuals in maintaining independence, safety and wellbeing within the community. Working collaboratively with service users, families, carers and multidisciplinary teams, you will develop and deliver personalised programmes tailored to meet individual needs.
Carry out comprehensive assessments of functional abilities and daily living needs in community settings.
Plan, implement and review individualised intervention plans to promote independence and quality of life.
Collaborate with service users, carers and professionals to coordinate effective care and support.
Maintain accurate and up-to-date case records in line with council and professional standards.
Provide advice, guidance and training to service users, carers and colleagues on adaptive strategies and equipment.
Contribute to service development initiatives aimed at improving community occupational therapy services.
