Join Wokingham Borough Council’s Adult Social Work Team and help empower adults to live independently and with dignity. As a Qualified Social Worker, you’ll work in a strengths-based, person-centred environment, supporting individuals with complex needs while promoting their wellbeing and autonomy.
Key Responsibilities:
Conduct strengths-based assessments to determine eligibility for care and support under the Care Act 2014, focusing on individual capabilities and community resources.
Develop and review care and support plans in collaboration with service users, carers, and multi-agency partners to ensure outcomes are met effectively.
Safeguard vulnerable adults by identifying risks, undertaking Section 42 enquiries, and working closely with the Adult Safeguarding Hub.
Support hospital discharges and transitions through the Hospital Liaison and First Contact Teams, ensuring timely and safe care arrangements.
Maintain accurate case records and prepare reports in line with legal and professional standards, including Mental Capacity Act assessments where appropriate.
Engage in continuous professional development, with access to training such as Best Interests Assessor (BIA), AMCP, and Practice Educator qualifications.
