As an Occupational Therapist with Leeds Community Healthcare NHS Trust, working within community respiratory rehabilitation services, you will provide specialist occupational therapy assessment and intervention. You will collaborate closely with service users, families, carers and multidisciplinary teams to develop and deliver personalised programmes that promote independence, respiratory health and safe use of equipment and adaptations.
Conduct comprehensive assessments of functional abilities and respiratory-related rehabilitation needs.
Develop, implement and review tailored occupational therapy intervention plans focused on respiratory rehabilitation and equipment use.
Work collaboratively with service users, carers and professionals to coordinate care and access to community resources.
Maintain accurate and timely clinical records in accordance with NHS and professional standards.
Provide education, advice and training to support patients, carers and colleagues in respiratory management and equipment use.
Contribute to service development and quality improvement initiatives within the occupational therapy team.
