Tripod Partners are looking for an RMN to join a Community Mental Health Team (CMHT) in Preston. The role involves caseload management, mental health assessments, and duty diary management for individuals with varying mental health conditions and severity.
Key Responsibilities:
Act as a keyworker for patients, managing a caseload with a range of mental health conditions.
Conduct mental health assessments and develop personalised care plans.
Provide medication management and support adherence to treatment.
Work within a multi-disciplinary team, liaising with professionals across health and social care.
Manage the duty diary, prioritising urgent assessments and ongoing care needs.
Support the implementation of the new CMHT transformation model.
Position Details:
Location: Preston
Duration: 3 months
Hours: Monday to Friday, 9:00 AM – 5:00 PM
Pay Rate: £25.55 per hour
Requirements:
Registered Mental Health Nurse (RMN) or Social Worker with relevant qualifications.
Experience in community mental health nursing and caseload management.
Strong skills in mental health assessments and medication management.
Experience with duty diary management and crisis response.
Knowledge of CMHT transformation models (desirable).
