Key Responsibilities:
Conduct assessments and develop support plans under the Care Act 2014, ensuring older adults receive tailored and effective services.
Safeguard vulnerable older people, managing safeguarding enquiries and working collaboratively with families, carers, and health professionals.
Promote independence and reablement, working with multi-disciplinary teams to support hospital discharges and community-based care.
Maintain comprehensive records and contribute to service improvements, demonstrating reflective practice and adherence to professional standards.
