As a Physiotherapist with County Durham and Darlington NHS Foundation Trust, working across inpatient, community and acute settings, you will provide physiotherapy services to adult patients within general medicine and musculoskeletal care. Operating at Band 6 or Band 7, you will act as a Best Interest Assessor and collaborate closely with multidisciplinary teams to deliver tailored therapy that supports recovery and improves patient outcomes.
Conduct comprehensive physiotherapy assessments of adult patients across inpatient, community and acute settings.
Develop, implement and evaluate personalised physiotherapy programmes based on clinical priorities and best practice.
Work collaboratively with patients, families, carers and healthcare professionals to coordinate care and support.
Act as a Best Interest Assessor where required.
Provide physiotherapy interventions in general medicine and musculoskeletal care.
Maintain accurate and detailed clinical records in accordance with professional and organisational standards.
