As a Speech and Language Therapist with Central and North West London NHS Foundation Trust, working with adults in community settings, you will provide specialist assessment and intervention for patients with neurological needs. Operating at Band 6, you will collaborate closely with patients, families and multidisciplinary teams to deliver person-centred care and optimise communication and swallowing outcomes.
Conduct comprehensive assessments of speech, language, communication and swallowing for adults with neurological conditions.
Develop, implement and evaluate personalised therapy programmes based on clinical priorities and best practice.
Work collaboratively with patients, families, carers and healthcare professionals to coordinate care and support.
Provide advice, strategies and support to improve communication and swallowing.
Maintain accurate and detailed clinical records in accordance with professional and organisational standards.
Contribute to service development, quality improvement and multidisciplinary team initiatives within the speech and language therapy service.
