Job Details:
Rehab Support Workers required to start ASAP for 6 weeks within the community rehab service. The post holders will provide support to clients under the guidance of Physiotherapists, Occupational Therapists, and other senior staff, helping to deliver care and rehabilitation plans.
Key Responsibilities:
Implement care and rehabilitation plans designed by therapists.
Report concerns, rehabilitation progress, and goal reviews to case holders.
Support daily observation-taking and identify changes in client conditions to prevent hospital admissions.
Attend weekly multidisciplinary team meetings to provide feedback and contribute to client progress.
Promote timely, safe discharges and support proactive case management.
Ensure accurate record keeping and administrative tasks.
Undertake autonomous and lone working in the community.
Use tact, negotiation, and motivational skills when working with clients and carers.
Involve clients and carers in planning and encourage self-management where appropriate.
Communicate effectively with the Integrated Community Service and wider MDT.
Working Hours:
Full time.
