Job details: Physiotherapist required to provide additional support. Keep Safe Stay Well Service (Community Falls Prevention Service) Short Term Support & Rehabilitation Team. Duties:
1) Carrying out a comprehensive Multifactorial Falls Risk Assessment in clients’ home environment which includes- taking subjective history, falls history, assessing their bone health and falls risk factors, medication history, cognition and memory, vision and hearing issues, continence, checking environmental risk factors, assessing their pain, joint range of movement and muscle power, balance and mobility, fear of falling, also taking outcome measures such as TUG test, 180 degree turn test, and Tinetti score to identify clients’ falls risk, finally formulating a patient-centred treatment plan to help reduce falls risks.
2) Carrying out physiotherapy assessment, and providing personalised falls prevention interventions including exercise therapy, advice, equipment/aids.
3) Taking part in Falls Prevention Talks/ Presentation in the community (as/when needed).
4) Collaborative working with stakeholders including GPs, health professionals (e.g., social workers, occupational therapists, social prescribers), and patient families.
5) Triaging referrals received from various sources including GP, Social prescribers, and self-referrals.
Working hours: Part time. 2 days per week.
