To take a lead responsibility for some complex assessments and undertake other complex assessments jointly with Community Care Assessors or Social Workers.
To offer specialist advice to the team on clients’ functional, psychological and physical abilities using own professional knowledge base.
To take responsibility to ensure that appropriate parts of service packages (eg equipment and adaptations) are processed and monitored, in liaison with other departments.
To provide leadership for junior staff, through supervision and appraisal.
To undertake assessments and write reablement plans, monitor outcomes and provide professional support to reablement workers
