The Rehabilitation Assistant (Band 3) at North East London NHS Foundation Trust will support intensive rehabilitation and therapy services at Grays Court Community Hospital and Goodmayes Hospital. Working closely with designated Physiotherapists and Occupational Therapists, you will follow individual treatment plans and provide care in both clinical and community settings, including home visits, to help patients achieve their rehabilitation goals.
• Support patients by following treatment plans designed by Physiotherapists and Occupational Therapists.
• Conduct home visits (x6 daily) to provide rehabilitation support in patients’ own environments.
• Assist with intensive rehab and therapy interventions across community and hospital settings.
• Maintain accurate records of patient progress and communicate effectively with the therapy team.
• Work collaboratively with multidisciplinary teams to ensure safe and effective patient care.
• Travel confidently to patient homes, with car driving preferable due to community-based duties.
