Job Details:
OT required to carry out Care Act assessments for individuals discharged from hospital into step-down beds (primarily nursing home settings, occasionally community rehab).
Work collaboratively with the multidisciplinary team (MDT) to support safe return home and arrange appropriate care and support packages.
Duties may include CHC assessments, mental capacity assessments, and functional assessments as required.
Must have relevant experience in community rehab and social services settings.
