Job details:  To cover vacancy. Occupational Therapist required for ICAT (Intermediate Care Allocation Team). A dynamic and self-sufficient Occupational Therapist is required to work within the Intermediate Care Allocation Team and Health in the Urgent Care, Health and Prison Service at Lancashire County Council. The post holder will be part of the ongoing development to expand services, with their knowledge of hospital discharge and Intermediate Care System. By using specialist therapeutic skills, the post holder will become an integral part of clients’ journeys to independence, providing the tools they need to progress to a happier, healthier way of life. Working alongside integrated teams of professionals from Health Social Care, Housing and voluntary sectors, the post holder will form a seamless level of care and understanding developing beneficial working relationships with colleagues and clients. The post holder will have passion and commitment in facilitating freedom and liberty for individuals who need to readjust to life after hospitalisation or crisis at home – they will be required to provide a superior level of support when it comes to restoring confidence for clients, allowing them to regain their zest for life and ability to live happily and safely in their own homes – the post holder will be making key recommendations and provide support to  Social Workers and Social Care Support Officers with assessments and practices putting individuals at the forefront of critical thinking – discretion and empathetic nature will allow clients to feel at ease when providing assessments in their home; as well as supporting those in 'discharge to assess' bed based care seeking opportunities for them to return home. Challenge – the post holder will be challenged on a daily basis, dealing with competing responsibilities and finding the right balance between various tasks – self-motivation and determination will allow the OT to breeze through the day, remaining flexible and decisive – we need someone who can communicate effectively with various care facilities such as care homes, crisis, reablement and domiciliary services, to provide beneficial education within their care roles – they will also provide support to the wider team using analytical skills to identify individuals who require ongoing interventions of a therapeutic, rehabilitation or reablement nature Critical Thinking – we want you to think outside the box and be a key problem solver by always being solution focused and striving for improvements where possible – you will have an exceptional understanding of what role OT plays within social care, knowing what level of support and skills are required for each individual – we want you to make valuable recommendations for use of the 'Discretionary Fund' where a commissioned service is not available to ensure all clients needs are met – you will also provide initial therapy follow ups which may include equipment checks or immediate specialist rehabilitation – your swift actions and critical thinking will ensure there are no delays to vital care, maintaining trust and satisfaction in the services we provide
Working hours: 37 hours per week. 08.45 – 17.00.
