Job details:  Community Adult Falls. On site/Community working. Tri borough Falls Prevention team comprising of Physiotherapists, Occupational Therapist and Rehabilitation assistants providing specialists falls assessments and interventions to patients who have history of falls, fear of falling or at risk of fall/s.  The service sees residents of Westminster, Kensington & Chelsea and Hammersmith & Fulham who are at risk of falls.  Clients are predominantly seen in clinics at various locations for a specialist falls risk assessments and evidence-based interventions including strength and balance exercise and education classes. The Falls Prevention service also see a small number of clients in their own homes for further assessments and interventions/ one-to-one rehabilitation where indicated.
A highly motivated, passionate, and enthusiastic team player is required with a positive attitude and commitment to providing excellent holistic care and interventions. Candidates preferred with previous experience of working within falls prevention service and have sound knowledge, skills, and confidence in carrying out a Multifactorial falls risk assessments, outcome measures, planning and implementing individually tailored treatment programs to reduce the risk of fall/s and have the ability to lead strength and balance classes alongside the rehab assistants. Will need to complete S1 training.
Skills/experience: Qualified HCPC registered PT. Experience with falls prevention service. Experience with Community Rehab. At least 2 years Post qualification. JD available on request:

• Be an active member of the clinical team, providing a specialist falls service to residents of Westminster, Kensington and Chelsea and Hammersmith and Fulham.
• To provide comprehensive multi-factorial falls assessment, treatment, and evidence-based programme of rehabilitation to all clients who have fallen as part of an inter-disciplinary service.
• To hold a clinical caseload of patients at risk of falls as well as those needing general community-based rehabilitation, delivering client centred, evidence-based physiotherapy interventions.
• Work within professional standards and clinical guidelines, promote best practice and to undertake all aspects of clinical duties as an autonomous practitioner including assessing and treating own caseload of patients/clients and maintaining professional documentation.
• Build up and maintain key relationships with referring services across CLCH including the other Falls and Bone Health Services in CLCH, Acute Falls Clinics, Community Rehabilitation Teams, Hospital at Home Team, Community Independence Service, GPs, District Nurses, A&E, Ambulance Services and other community service providers to facilitate appropriate referrals and an understanding of the role of the Physiotherapist in the Falls Prevention Service
• Be responsible for the supervision of junior staff, Rehabilitation Assistants and Physiotherapy students as the opportunities arise.
• Work in a variety of settings including clinics, client’s own home, community centres, extra care sheltered housing and continuing care homes.
• Support self and service development through active participation in Continuing Professional Development (CPD) and by participating in any audit or research being undertaken by the service.

Role is more office based than ward based – post holder will need to be on the ward daily especially at board rounds for all the pathway 1 patients to help with the discharge been able to identify the patients and prioritise them as they need to be discharged. A lot of computer work and telephone conversations. Communicating with the community team I.e. Care agency / DN team / Redcross / My care My way / and other services. Ideally the candidate should be experienced in hospital discharges and able to problem solve and work closely with the MDT within the hospital. Skills/experience required: 
1) Knowledge of community services, Knowledge of hospital discharge processes, excellent clinical risk assessment. 
2) knowledge of nursing and functional community equipment, knowledge of referral pathways 
3) Excellent communication Skills and efficient time management, To be able to manage a busy caseload, computer skills.
4) Attend the first-floor meetings 8.30am and board round at 12 - this is to cover the first floor
5) To log in to Cerner daily and follow up on the patients - awaiting care packages Ie identify the patients that can be discharged once MFFD
6) Daily meetings on Teams
7) To be able to communicate and hand over to the community Home first team Daily 
8) To be able to follow up with the Therapist on the wards and 
9) Documentation on the patient’s hospital notes 
10) Identifying any issues or raised or concerns with the patients NBA
