Job details:  Urgent Community Response / Rehabilitation Home Pathways. To start asap. Initial assessment, OT assessment, rehab care plans, case manager and part as patient of the MDT, equipment assessment, manual handling and review, cognitive assessment joint working with PT. Skills/experience: Experiences of working with older adults, worked as part of a rehab team, community or hospital experience. Working hours: Monday to Friday. 37.5 hours per week. Can be long days or short / possible weekends.
