To manage a caseload of Intermediate Care Unit based patients with complex needs, using specialist skills and evidence based/patient centred principles to assess, plan, implement and evaluate interventions.
To supervise/train inexperienced junior staff and rehabilitation assistants working within the service.
To provide supervision of students on practice placement.
To participate in the planning, delivery, and evaluation of Intermediate Care services within a designated area/team, holding responsibility for defined projects.
To be an active member of the inter-disciplinary Intermediate Care team in the provision of patient-centred care.
To develop and provide clinical expertise in a designated specialism and participate in the dissemination of evidence-based practice across the service.
