Job details:  Out of Hospital Care. Integrated Discharge Team. Previous experience in IDT preferable, ability to work as an autonomous practitioner. Keen to work with patients and supporting timely discharge in an acute setting.
Working hours: Full time. Monday to Friday, 08.00 – 16.00. Thursday-Monday full or part time, to include weekend working.
